A US Passport application was filed by RC Middleton, fruit buyer living in Crescent City Florida, on January 11, 1918, for the purpose of traveling to Cuba aboard the P&O Steamship Line. The application was filed in Jacksonville, and lists Key West as the port of embarkation.
The application lists his father as WB Middleton (incorrect - it should be WD Middleton) and shows his birthplace as Hortense Georgia. It also gives his birthdate as 16 Nov. 1876.
RC Middleton is described as 41 years old, 5 foot 8 and a half inches tall, brown eyes, brown hair, straight nose, medium mouth, fair complexion, and face "round-full".
It also shows the written affidavits of a Jacksonville Realtor named J.J. Heard who says he's known Mr Middleton personally for six years, and a Mr. T.E. Godfrey of Sawyer-Godfrey Fruit Company. Also included is an additional affidavit from a Mr. Raleigh C. Dowling described as life long friend who testifies that Mr Middleton was born in Hortense GA.
