The funeral for Ashley Danielle Polk, 5, will be at 2 p.m. Thursday at Owen Funeral Home, 5317 Dixie Highway. Burial: Bethany Cemetery. Visitation: 2-9 p.m. Tuesday and 9 a.m. -9 p.m. Wednesday. She died Sunday at Kosair Children's Hospital after a parked truck slipped out of gear and rolled over her. Survivors: her mother, Stacie D. Heavrin; her father, Allen E. Polk; a brother, Cory A. Polk; grandparents, Mr. and Mrs. Larry Heavrin of California, Mr. and Mrs. Roy Garner and Mr. and Mrs. William Polk; and great-grandparents, Mr. and Mrs. John Heavrin, Mr. and Mrs. Noble Hall and Mrs. Ruby Weber.
