LCDR Jeffrey Forrest Smith was a decorated Vietnam Veteran attached to Helicopter Attack ((Light) Squadron Three, HA(L)-3 in 1967-1968. He was awarded 3 Air Medal-Strike/Flight Awards and a Navy and Marine Corps Commendation Medal (with Combat V). Approximately 8-9 years later, he was OinC of HSL-33 Det 1 aboard USS Sterett when the heliopter he was flying crashed at night in Pacific Ocean. Other crewmen were rescued but neither the helicopter nor LCDR Smith's body were recovered.
