National Defense Service Medal, Global War On Terrorism Service Medal, Humanitarian Service Medal, Army Service Ribbon, Overseas Service Ribbon, Combat Infantryman Badge, Driver and Mechanic Badge with Driver-Wheeled, Served in a Designated Imminent Danger Pay Area, Served in Iraq, Iraq Campaign Medal with Bronze Service Star, Overseas Service Bar.

Survived by Mother, Christina Crittenden; Father, Jerry Creek.

Services - Mark Griffith Memorial Funeral Home - Westwood Chapel in Tulsa and will be held on Monday, July 9, 2012, at 10 a.m. at the Mark Griffith Westwood Chapel with Interment to follow at 1 p.m. Monday at the Ft. Gibson National Cemetery.
