Nancy Austin "Cisssy" Smith Perkins was born 16 May 1878 in Fayette County, TX to James Thomas "Tom" Smith and Cornelia Redd. In 1915 at the age of 38, she married George Riley Perkins. They had two children: Annie Myrtle Perkins and an infant girl. During portions of their marriage they lived apart. George is listed as a pauper in the 1930 census and in his death certificate.

Nancy's facial deformity may have been caused by the use of the drug calomel. This deformity was often the source of family discussions. As late as 2016, it was confirmed by her great-niece, Alma Ashley Urbanek.

Calomel was a widely prescribed drug at the time for a variety of ailments such teething, dental pain, constipation, and more. What was not fully understood at the time is that it contains high levels of mercury and the dangers of such use.
