Hazel was a strong member of The Church of Jesus Christ of Latter-day Saints. She faithfully served in many callings and had a very strong testimony.
Hazel served in the U.S. Navy from 1956-57. She worked in the hot lunch program for many years, and then enjoyed caring for many children who all called her "Grandma Hazel."
Hazel is survived by her husband, Dan; four sons: Steven (Janalee) Banks, Rocky Banks, Rusty (Tammie) Banks, Kim (Julie) Banks; one daughter, Alona Banks; nine grandchildren: Todd, Mitchell, Jacob, Katrina, Shannon, Sydney, Emily, Kayley, and Tanner Banks, all of Spanish Fork. She is also survived by a sister, Mildred (Gene) Waits, and one brother, Hughdon Parker, both of Georgia. She was preceded in death by her parents; one sister, Agnus; two brothers, Henry Galloway and Hubert Parker; one grandson, Brady Banks; and one nephew, Henry Thomas Galloway, Jr.
Funeral services will be held Monday, August 18, 2003, at 1 p.m. at the Lake Shore LDS Chapel, 5916 South 3200 West. Friends may call on Sunday evening, August 17, 2003, from 6-8 p.m. at Walker Family Mortuary, 187 South Main, Spanish Fork; and on Monday, at the church, from 11:45 a.m. to 12:45 p.m., prior to the services. Burial will be in the Spanish Fork City Cemetery.
The family would like to thank Dr. Gerald Stephanz, Dr. John Frischknecht, the many nurses and staff members at UVRMC, and wonderful staff at the DiVita Dialysis Center for their wonderful, loving care.
