The Breckinridge News Wednesday, 5-12-1915:

David Mattingly died Tuesday morning after an illness of several months. He suffered with his heart and had several bleeding spells. His death has been expected for sometime and while he was a great sufferer he was conscious to the last. The funeral will be held from the St. Rose Church this morning at 9:30 o'clock. Twenty-two years ago Mr. Mattingly married Miss Margaret Mattingly. Seven of their twelve children are living, the oldest one, Carl Mattingly is eighteen years of age and for the last year he has taken responsibilities of the family. He cared for his father, without a cross during his entire illness and is a devoted son to his mother. Mr. Mattingly left his family $2,000. He carried insurance in fourteen companies, Mr. Mattingly worked at the L. H & St. L. R. Shops. Besides his immediate ones, he leaves his mother, Mrs. Eliza Mattingly, one sister, Mrs. Carl Benton, of Louisville and three brothers, Fraize Mattingly of Owensboro, James Mattingly of Marshall, Texas and Lewis Mattingly of Kansas.

DAVID W. (an obvious error) MATTINGLY, aged thirty-nine years (S/B 42), a boiler maker for the Louisville, Henderson and St. Louis Railroad Company, died at his home in Cloverport, Tuesday morning at 4 o'clock. Mr. Mattingly had been ill of a complication of diseases for about six weeks. He is survived by his widow and eight children, as also his mother, Mrs. Elizabeth Mattingly, three brothers, Fraize Mattingly, of Owensboro; Louis Mattingly, of Kansas; James Mattingly, of Marshall, Texas, and one sister, Mrs. D. C. Benton, of Louisville. Funeral services will be conducted from the Catholic church at Cloverport at 9 o'clock this morning, with the interment following in the church cemetery. The Woodmen of the World, of which the deceased was a member, will hold their burial services at the grave. Fraize Mattingly, an uncle and aunt, Mr. and Mrs. Robert Mattingly, went to Cloverport Tuesday to attend the funeral.

Note: His headstone shows him born on the 12-12-1873, the 1900 census shows May 1876. He was aged six on the 1880 census and his correct date of birth was 12-12-1873.
