Several family trees show his date of death as September 8, 1943 in Avery North Carolina. There is a listing for a Charles H. Johnston in North Carolina death records for that date. Per the 1900 United States Census he was born February 1878 in Washington, D.C.

His full name was Charles Haven Ladd Johnston. Some of his books are available as reprints on various book websites.
