First Lieutenant Donnelly was killed when the Bell OH-58A Kiowa helicopter he was piloting crashed during a routine training flight on Mount Toro about 15 miles south of Palm Springs. He was flying alone from Monterey, California to Salinas, California.

He is not listed on the Vietnam Memorial because he died stateside. He is represented on the Remembrance Rug by symbol #5784.
