Hazen served as a Staff Sergeant & Ball Turret Gunner on B17-G "Boomerang" #42-102610, 602nd Bomber Squadron, 398th Bomber Group, Heavy, U.S. Army Air Forces during World War II.

He resided in Middlesex County, Massachusetts prior to the war.

He enlisted in the Army on September 21, 1942 in Boston, Massachusetts. He was noted, at the time of his enlistment, as being employed as a Carpenter and also as Single, without dependents.

B17-G #42-102610 took off, with a crew of 9, from Nuthampstead, England on a bombing mission over Bischofsheim, Germany. Shortly after takeoff this B-17 was seen with smoke trailing from the engines and to eventually crash into the English Channel. Upon impact with the water the B-17 exploded. Rescue operations found "1" crew member struggling in the water ( 1st Lt. Walker ) but they were unable to rescue him before he perished. The remains of "2" other crew members were also later found ( S/Sgt.'s Fox & Stewart ). The remains of the rest of the crew were never found.

Hazen was declared "Missing In Action" in this crash during the war.

He was awarded the Air Medal with "3" Oak Leaf Clusters.
