CRASH INFORMATION ON U.S. ARMY HELICOPTER CH-47A TAIL NUMBER 66-19069
Accident Summary: The aircraft was flown following a re-inspection of the completed work done after the replacement of two engine gear boxes to transmission drive shafts. The helicopter was observed to explode in mid-air and fall to the ground burning. The cause has been determined to be desynchronization of rotor blades due to lack of lubrication in the combining transmission. Crew members lost in this incident included CW4 Dale A. Crull (KIA), CW2 Robert L. Brown (KIA), SP4 Jess W. Creason, (KIA), SP6 Anejelo J. Aprilliano (KIA), and SP5 William C. Deweese (KIA). Passengers included SP5 Michael P. Stiles (KIA), SSG Larry A.
