William Spencer Middleton was the second son of Alexander Goodby Middleton and his wife Mary Polly Townsend Middleton.

He married (1st) Mary Ann Drawdy, the daughter of William and Mary Drawdy, sometime prior to 1849, the year that their first child was born.

William was still living in Militia District 22 in McIntosh County for the 1850 census (Shellman Bluff area), but by 1860 he had moved to Wayne County where he practiced medicine. Family historian Beth Engel states in her book that he received his medical degree in Baltimore, Maryland.

Dr Middleton married his 2nd wife, Ruby Lavina Mattox on March 26, 1876, exactly six months after the death of Mary Ann. William Epping Middleton (who also became a doctor) was William and Ruby's only child. Ruby died exactly 18 days after William died.

All three of William Spencer's sons went on to be extremely successful and prominent men in their communities.

Eldest son Benjamin Osgood Middleton was the father of eleven children by his first wife Mary Jane Howard and second wife Rebecca Learie Benton. He was also one of the largest landowners this family has ever produced.

Middle son Dilworth ultimately fathered ten children, many of whom settled in Putnam County Florida; making Dr. W.S. Middleton the ancestor of the many Middleton progeny in Central Florida.

William's youngest son, Dr. William Epping Middleton, settled in Bradford County Florida in about 1918. He and his wife Agnes Dowling were the parents of four children.

Doctor William Spencer Middleton, and his wives Mary Ann and Ruby, are all buried at the Hortense Cemetery in Hortense Georgia.
