During Operation TRACK DUSTER in Phuoc Tuy Province on 8th October 1968, 12 Platoon were checking signs suggesting that an enemy camp was in the vicinity. A scout had found a bag of clothing close to a track and the platoon commander and the scouts moved a short distance to investigate. From a distance of thirty five metres, the enemy opened fire with small arms and three large command detonated mines. Keith and another soldier, Private Kevin Brewer, were killed and six others wounded.
