Ellsworth served as a Private First Class, 180th Infantry Regiment, 45th Infantry Division, U.S. Army during World War II.

He resided in Washington County, Maryland prior to the war.

He enlisted in the Army on July 29, 1943 in Baltimore, Maryland. He was noted, at the time of his enlistment, as being employed as a Farm Hand and also as Single, without dependents.

Ellsworth was "Killed In Action" in Italy during the war.

He was awarded the Purple Heart.
