Second Lieutenant O'Brien was one of four airmen killed when U.S. Army Air Corps B-25D Mitchell #41-30349 crashed into San Pablo Bay, two miles offshore after takeoff from Hamilton Field, in Marin County, California.

The twin engine bomber was being ferried to Hickam Field in Hawaii when the accident occurred.
