Cause of death was "Basilar Meningitis." He was the son of T. D. Butler, born in Indiana, and his mother's maiden name was [Mary] Asher. Richard Butler was the informant. Burial was at Odd Fellows Paragon [Friendship Park] Cemetery in Paragon, Morgan County on January 15, 1913, Nutter and Funkhouse of Martinsville, undertakers.
