While "serving as the point on a reconnaissance in-force operation near Lai Khe, SP4 Mendias sighted a well-fortifed enemy position, simultaneously warning his comrades. With complete disregard for his own personal safety, he braved hostile fire as he neutralized one enemy position with devastating automatic rifle fire while standing in the aggressor killing zone. Undaunted by the enemy fusillade, he then threw hand grenades to mark the position further. He was mortally wounded as he attempted to move to a more secure position."

He served with Company B, 1st Battalion, 22nd Infantry, 1st Infantry Division. Recipient of the Combat Infantryman's Badge, Bronze Star with "V" device and Oak Leaf Cluster, Purple Heart, Good Conduct Medal, National Defense Service Medal, Vietnam Service Medal, Vietnam Campaign Ribbon, the Republic of Vietnam Military Medal and the Gallantry Cross with Palm.

His name is located on on Panel 30W, Line 14 of the Vietnam Memorial.
