As of the 1910 census there were three children, listed in this order:
1. Nell P (age 9 yrs)
2. Henry W (age 9 months)- link below
3. May (age 6 yrs)
May Paxton Puryear obit references being married a year and there being a baby, which means the baby would have been born very shortly prior to her death in March 1904. This could be the listed #3 child "May"
The 1910 census says they had been married 2 years; Kentucky marriage records indicate Peter Puryear & Brunella L Paxton married May 1907. The census also says Nellie has 2 of 2 children living; but, the children's ages do not correlate with this; i.e., Nell P would have been born in 1901.
However, the 1920 census reflects Nell P born in 1909. This makes more sense - In the 1910 census Nellie's 2 children listed first, followed by May's child.
The 1920 census lists an additional child:
