M. Annette (Moore) Forkey, 84, passed away on Sunday, May 27, 2012 at home in Tonopah, Ariz. Born on February 23, 1928 in Fair Haven, Vt., she was the daughter of the late Henry B. and Bessie W. Moore. She was an active member of Zion Bible Church in York and pianist there over 40 years. She taught Sunday school and was an office assistant as well. She was preceded in death by husband, Clyde Owen Forkey of Benson, Vt. She is survived by two sons and a daughter: Richard O. Forkey and wife Connie Chaplin of Weaverville, N.C., Wade D. Forkey and wife Christine of York and Ann M. Cerda and husband Francisco J. of Tonopah, Ariz. She is also survived by three grandchildren, Francisco O. Cerda and fiancee Katheryn Platner of Tonopah, Ariz., Gina M. Gardner and husband Jon of San Diego, Calif. and Jennifer R. Cerda of Tonopah, Ariz. Following cremation, a graveside service will be held at Stony Brook Mennonite Cemetery on Monday June 11 at 2 p.m. Officiating will be Pastor Ben Woodring of Zion Bible Church. In lieu of flowers, contributions may be made to Zion Bible Church, 1550 N. East St., York, PA 17406. The Etzweiler Funeral Home and Cremation Service, 1111 E.
