Originally buried in a group of 6 graves belonging to the 2nd Div. in a hospital graveyard 4 miles NW of Pine Mt. - Levi Smith, Henry O. Wilds, George Turner, Geo. Pendergrast, T.P. Thomas, H. Allstetter.

In the re-interment record, death date was June 18, 1864, but 18 crossed out and replaced with June 11 when name was reconciled with AG report.

American Civil War database lists death date as June 11, 1864. Died from wounds.

In the OR report, Col. Strickland commanding the 3rd Brigade reported that on June 11, "rebels shelled our position [Davis house]; loss, Fiftieth Ohio, 1 wounded [could be T P Thomas who died June 12]; Twentieth Kentucky, 2 killed [Pendergrast and George Turner], 1 wounded".
