Cole was the son of Samuel and Annie Margaret (Belle) Bledsoe McMillian. He married Anna Mayo in 1909 in Platte City. She died in a house fire in 1914. He then married Lena Grable. She later died from TB at the age of 25. He then married Julia Dick Caples. He had three children with Anna Mayo. They were Elmer Cole McMillian, James Russell (Bob) McMillian , and Amy Grace Newman. He then had one daughter with Julia named Anna Grace Smith.
His siblings were Luella Clemens Lott, Hattie Ann Winder, Claire Winder, A.B. McMillian, Robert Lee McMillian, Beatrice Miller, Effie E. Wilkerson,Kate Eskridge, and Louise Coy.
