Services for John C. (Jay) Forman, 80, a retired dairy farmer and former St. Joseph resident, will be at 2 Thursday at Ebenezer Methodist Church, Highway 169, east of St. Joseph. Interment will be at Ebenezer Cemetery. The body is at Sheil Mortuary, 6606 Independence Avenue, Kansas City. Mr. Forman died Monday night at St. Joseph Hospital in Kansas City. Born at Kingwood, W. Va., he had lived in Raytown 25 years, making his home with a daughter, Mrs. Josephine Guiter, 7603 Hawthorne. He had been retired ten years. In addition to his daughter, survivors include another daughter, Mrs. Lucille Newton, Morgantown, W. Va., and two sisters, Mrs. Estella Harriman, Morgantown, and Mrs. Lyda Fairmont, living in West Virginia.
