Helen Ilene Formway, formerly of San Bernardino where she was born, died June 15, 1980 in Carmichael at the age of 67. She was of the Protestant religion and was survived by her two daughters, Joy Blake of Highland and Shirley Tuter of Carmichael, one son, Howard Chapman of Sacramento, two sisters, Lucille Beebee of Mentone and Lorraine Ross of Seattle, Washington, three brothers, Guy and Claud Evans of San Bernardino and Morley Evans of Rialto, 8 grandchildren and 5 great-grandchildren. Graveside services will be at Montecito Memorial Park on Thursday at 10 a.m. with Mark B. Shaw Baseline Chapel in charge of the arrangements.
