Maj. John B. Boltz, 66, of 6540 Oshkosh av., a retired army officer and owner of an insurance brokerage firm at 330 S. Wells st., died last night in Resurrection hospital. He leaves a son, John Denton Boltz; two sisters; and four brothers. Services will be held Wednesday in the chapel at 6754 Northwest hwy.
