Sid Underwood was the son of Jasper Anderson Underwood and Mary Lucy Meadows.

He was married to Lydia Alice 'Attie' Neluss on Jan 1, 1897, probably in Chickasaw Nation, Indian Territory, now Oklahoma. They adopted Bertha Campbell after Bertha's mother Nora (Sidney's sister) died.
