Samuel served as a corporal in Company A of the 98th PA Infantry, mustering into service on 22 Aug 1861. He suffered an injury at the Battle of Malvern Hill, Virginia on 1 Jul 1862 and died of gastroenteritis on 2 Nov 1862 in a US Army hospital in Portsmouth Grove, RI.
His original burial was in the Philadelphia IOOF cemetery on 5 Nov 1862. Although many soldiers' graves were moved in 1885 to the Philadelphia National Cemetery, it appears that Samuel remained with his family. The Phillips family graves were moved to Lawnview cemetery on 24 Aug 1951.
