TULSA, WORLD NEWSPAPER OBITUARY, March 10, 1958. Mrs. Lucille Blair, 44, died Sunday morning in a Tulsa hospital. A Tulsa resident 25 years, she was the wife of Sam Blair, owner of Mid-Continent Recreation center, 1314 W. 17th St. She was born at Harrah. She lived in Wagoner before moving to Tulsa. Other survivors include two sons, both of the home; her parents, Mr. & Mrs. Phillip Kozak, Wagoner; four sisters, Mrs. Teresa Allred, Stratford; Miss Martha Kozak & Mrs Frances Ballou, both of Boston, MA. & Mrs, Helen Stansill, Tulsa, three brothers; Frank, Clemmons, & Phillip Kozak, all of Tulsa, Services will be held at 3:30 Tuesday in the Tabernacle Baptist Church with interment in Memorial Park cemetery.
