THE LAMAR COUNTY ECHO, 29 Jan 1987: 'Lavana Goff Forman, widow of Hubert Clifton (Chick) Forman, Rt. 1, Powderly, died at 5:45 p.m. Sunday, 25 Jan, at McCuistion Regional Medical Center. Services were held Thursday, 29 Jan, at Fry and Gibbs Funeral Home Chapel with the Rev. Mike Howard, pastor of Bounds Baptist Church, and the Rev. Clarene R. Hamilton, pastor of Mt. Olive Baptist Church, officiating. Burial was in Long Cemetery. Mrs. Forman, was born 22 Dec 1913, in Lamar County, a daughter of John and Nora Holland Goff. She attended Lamar County schools and married Mr. Forman on 18 Sep 1943, in Fort Worth. He died 12 Nov 1982. They lived in Fort Worth until 1962 when they moved to Weatherford, where they lived until 1972 when they moved to Powderly. She was a member of the Bounds Baptist Church and its ladies adult Sunday school class. Surviving are a daughter, Phyllis Gail McCormack of Garland; two sons, Billy H. Forman of Lompoc, Calif., and John F. Forman of Powderly; three grandsons, Michael Forman of Lompoc, William Forman of Lompoc and Clayton McCormack of Garland; four sisters, Nora Dale Anderson of Paris, Mattie Lue Whitaker of Paris, and Clara B. Brownlee of Garland; and a brother, J.H. Goff of Fort Worth.'

From an unidentified and undated Paris newspaper clipping: 'Funeral services for Lavana Goff Forman, Rt. 1, Powderly, were conducted Thursday in Fry and Gibbs Funeral Home Chapel with the Rev. Mike Howard and the Rev. Clarence R. (Bob) Hamilton officiating. Burial was in Long Cemetery. Bearers were R. G. Cobb, Ken Poole, Orville A. Hulsey, Johnny Exum, Marion D. Burrow and Randy Foreman. Mrs. Forman died at McCuistion Regional Medical Center.'
