Indiana State Board of Health Certificate of Birth No. 4037 for Van Buren Township, Brown County, records that Not Named [Crouch], white female, was born alive about 12:11 a.m. on September 18, 1908 in Van Buren Township. Her father was Joseph W. Crouch, white, age 38y, a farmer, and born in Indiana, and her mother was Lillie M. Shepherd, white, age 31, and born in Indiana. The birth was legitimate, and was her 9th birth with 8 living.

Indiana State Board of Health Certificate of Death No. 203, page 15 for Van Buren Township, Brown County, records that Not named Crouch, white female, age about 1 hour, died about 1:42 a.m. on September 18, 1908 in Van Buren Township. The cause of death was "Atelectasis Pulmonum." She was the daughter of Joseph W. Crouch, and Lilly M. Shepherd, both born in Indiana. The informant was her father Joseph, and she was buried at Hiram Crouch Cemetery on September 18, 1908, no undertaker was noted.
