Phyllis was born May 7, 1943, in Talmage, KS, to Hugh and Berneice Allen.
Phyllis is survived by her husband Melvin Perkins, son Lee (Sarah) Perkins of New Castle, CO, and daughter Connie (Rick) Turner of Glenwood Springs, CO. She is preceded in death by her parents, brothers Homer (Bud) and Jackie Allen, and her sister Joanie Allen. A memorial service will be held Saturday, December 15, 2012, at 1:00 pm at the Farnum Holt Funeral Home Chapel in Glenwood Springs, CO. In lieu of flowers memorial contributions can be made to Hospice of the Valley P.O Box 3768 Basalt, CO 81621.
