Born in Spartanburg County, she was a daughter of the late Perry Ballew and Onie Shields.
Hazel is survived by two daughters, Carolyn Tara (Don), of Greenville, and Mary Kathryn Adams, of Greenville; a son, Gus Allan Adams, Jr., of Chapel Hill, NC; three grandchildren, Jessica Tara of Shanghai, China, Jennifer Tara of Charlotte, NC, and Christopher Daniel Adams, of Greenville; two great grandchildren; two brothers, Kenneth Shields and Wayne McKelvey (Cathy); and two sisters, Joyce McKelvey and Sandra Kay Boucher.
In addition to her parents and loving husband, she was preceded in death by a daughter, Rhonda Paulette Adams; two sisters, Hilda McKelvey Shield and Floree Wallace; and a brother, Hugh McKelvey.
