Dave was born on September 1, 1945 to the late Evelyn Ellen Holden Ervin and Howard Merle Slade in Danville, IL. He was raised by his mother and late step-father Albert Ervin.
Dave was preceded in death by his brother Andy Slade, half-sister Carol Elmore and half-brother Steve Ervin. He is survived by his daughter Kara Leigh (Jim) Confer of Shorewood, IL, son Jamie Andrew (Caitlin) Slade of Roselle, IL, brother Tom (Kathy) Slade of Lilburn, GA and half-brother Gary Ervin of Sullivan, IL.
Dave attended Diamond Grade School and graduated from Oakwood High School in 1963. He joined the Navy in 1966 and was assigned to Naval Aviation with Attack Squadron 56 based at Lemoore Naval Air Station in California. He was deployed to Vietnam on the Carrier Ranger once and the Carrier Enterprise twice before being discharged honorably in 1970. He returned to Danville and worked for Inter-State Water Company for 16 years. While working at the water company he attended DACC, graduating in 1979. He then worked for Illinois Power for another 16 years, retiring as Assistant Gas Superintendent. Per his wishes there will be no visitation.
Interment will be at Sunset Memorial Gardens on Wednesday, June 21, 2017, with full military honors.
