CENTERVILLE, Ind. – Donald A. Cook, age 83, of Centerville, died Sunday, May 1, 2016, at Reid Health. Born September 30, 1932, in Rogersville, Tennessee, to William Clifford and Hazel Couch Cook, Donald lived in Centerville most of his life. He retired from Johnson Sheet Metal where he was a truck driver. He was a member of the First Baptist Church in Centerville, where he had been a deacon and a trustee. Donald was one of the original members of the Whitewater Valley Amateur Radio Club. Survivors include his wife, Bertha Creech Cook, to whom he was married on October 1, 1951; son, Donald J. (Darlene Black) Cook of Centerville, Indiana; grandson, Jonathan Cook of Woodstock, Georgia; and many friends. He was preceded in death by his parents; grandson, Gregory Cook; and brother, Crandall Cook. Visitation for Donald A. Cook will be from 4:00 to 7:00 p.m. Wednesday, May 4, 2016, at Mills Funeral Home, 405 East Main Street, Centerville. Funeral service will be at 1:00 p.m. Thursday, May 5, 2016, at Mills Funeral Home with Rev. James Girdley officiating. Burial will be in Glen Haven Memorial Garden.
