September 16, 2016 - Calvin's middle name provided by findagrave member LSJRJT.

According to Lundy's obituary, which I have transcribed below, he was born in Harrisonville, Iowa. Harrisonville is a ghost town in Plymouth County, Iowa.

This place of birth was also the one given on Calvin's death record at the Wapello County Court House. Also, on that death record, his father was named as Marion Weeks, born in Missouri, and his mother's maiden name was Catherine E. Shubundy, born in Kansas. The informant of this information was his son Noah W. Weeks, of Ottumwa.
