Services will be 1 p.m. Wednesday in Dawson & Wikoff Colonial Chapel North with visitation 6 to 8 p.m. Tuesday. Burial will be in Maroa Cemetery.
Mrs. Fornwalt was born March 16, 1898 in Texico in Jefferson County, daughter of Charlie and Hattie Gillman Smith. She was a member of Christian Fellowship and a homemaker. She married Guy Fornwalt on Dec. 22, 1915, in Mount Vernon. He preceded her in death on Feb. 28, 1983.
Surviving are her daughters, Nellie Fornwalt and Lila Fornwalt, both of Forsyth; Mrs. David (Laura Lea) Byers of Decatur; sons, David Fornwalt of Phoenix, Ariz.; Phillip Fornwalt of Decatur; Donald Fornwalt of Downey, Calif.; 24 grandchildren; 35 great-grandchildren; four great-great-grandchildren.
She was preceded in death by her husband, parents, three sons, one daughter, four brothers and four sisters.
