I tried everything I could think of and cannot find her in any death index, under any name, anywhere. She is buried next to her father with the name of Eunice Davis Smith on her gravestone.
Her husband was on the 1940 census listed as divorced. He is buried in a different section of the same cemetery.
It is possible that she could have remarried after being divorced but was buried with the name of her first husband on the gravestone. I have seen that happen before, and if so, she would be in a death index under a different last name.

I have her name as Lizzie Una or Unita Davis, b. 1889 Oregon, d. 26 Sep 1941 Portland, Multnomah, Oregon, married Leslie Alva Smith, b. 1881 Kansas, d. 4 Jul 1942 Portland.

After receiving the above info Jeffrey discovered I was then able to search for Elizabeth via her death date and discovered that she went by and died as Venita Smith which she is listed as in the State of OR death index as well as in her funeral notice in the newspaper. It is quite possible the newspaper and whoever gave info on her to authorities gave them incorrect information, or she took on the name of Venita as a nickname at some point in her life?? She is listed as Una on both the 1920 and 1930 census in Portland.
