He was a WW II veteran having served with the U.S. Army from 1940-1946. He received the Pearl Harbor and the American Theater of Operations Victory Medals.
He married Sylvia Marie Kleefstra on Feb. 7, 1970, in Winchester, Virginia. She survives in Danville.
Also surviving are a daughter: Martha Patricia Hayes of Wisconsin; a brother, Vernon (Marjorie) Hayes of Claremont, California and a sister, Mary Jean (Jan) Rus of Laguna Hills, California.
He was preceded in death by his parents and two brothers.
Dr. Hayes attended grade school in Provo, high school in Washington, D.C., and pre-med at Brigham Young University in Provo. He graduated from medical school at George Washington University of Washington, D.C., in 1940. He became board-certified in physical medicine and rehabilitation, specializing in psychiatry, in 1947.
