Lewis Patrick enrolled on October 28, 1861. He served as a Union Private with D Company, 45th Kentucky Mounted Infantry Regiment Division, under Capt W.S. Adams, he was shot at the Battle of Salyersville (aka Battle of Ivy Point Hill). He was taken to a hospital in Lexington where he passed on May 1, 1864 from typhoid fever, at age 47. In a story written in the The Patrick Family Volume II by the Magoffin County Historical Society, it said when his wife Lucy (Adams) was informed of his death she along with Arty Sayler Dykes jumped on a horse and rode 2 days to Lexington only to learn he had already been buried.

Lewis two sons Levi and Irvin also served during the Civil war. Irvin followed his fathers footsteps and served with the Union while Levi choose to serve with the Confederates. Both sons survived.
