Residence: Box 189, Miller Valley, Prescott, Arizona. Spouse: Mrs. Myrtle Epperson. Occupation: Clerk. At the time of his death, 12:55 A.M., January 6, 1931 he was age 35 years, 5 months, 12 days. Prior to death he spent five days in the U.S. Veterans Hospital, Whipple, Yavapai County, Arizona. Cause of death: Peritonitis, general; contributory: Ileus, Paralysis; Appendectomy, all of which are dated January 4, 1931. The Informant for the Certificate was I.D.
