Carolyn S. Calkins, Mrs. Grant R. Calkins (nee Carolyn Sue Webb), 3734 Glencagica Drive, Tarzana, died Saturday morning at the Medical Center of Tarzana from leukemia.

She was born June 12, 1941 in Sheldon, Missouri and moved to Burbank in 1953. She was graduated from John Muir Junior High School and Burbank Senior High Scool in 1959. In 1963 she received a Bachelor of Science in Public Health from UCLA. She was married to Grant R. Calkins June 23, 1962.

Survivors are her husband, Grant; nine-year-old daughter, Sandra Lynn; her mother and step-father, Margaret and William Gawthrop of Camarillo; a brother, Charles Webb of Van Nuys; a cousin, Mrs. Leo Palmer of Camarillo,; and aunts and uncles and cousins in Missouri. Her father Harold Webb died in 1947.

Services 10:30 Tuesday, June 6, Church of the Hills, Forest Lawn, Hollywood Hills. Visitation from 4 to 9 p.m. on Monday. Forest Lawn Mortuary.
