December 31, 1912, Charles Allen, Miner, age 30 years, married with two children, American, was fatally burned by an explosion of powder at the Averill No. 1 Mine.
There were no eye-witnesses to this accident, but it appears from the examination made that the decedent at the time of the accident was pouring powder into a paper cartridge from a nearly full keg which he held under his arm, when the powder was ignited by a carbide lamp that he had on his head, causing the powder to explode, burning him so badly about the face and body that he died January 3, 1913.
