Born Nov. 19, 1887 in Palmyra to Andrew William and Annie Matilida Johnson Johnsen, she married David R. Boyack on Nov. 18, 1908 in the Salt Lake LDS Temple. He died on Nov. 1, 1976.
Mrs. Boyack was an active member of the LDS Church and was a member of DUP.
Survivors include four sons and one daughter, Glen J. Boyack, William R. Boyack, Spanish Fork; E. Dean Boyack, Orem; D. Ross Boyack, Price; Mrs. Lawrence H. (Phyllis) Jones, Phoenix; 12 grandchildren; 16 great-grandchildren; one great-great-grandchild.
Funeral services will be conducted Monday at 1 p.m. In the Spanish Fork 2nd-9th Ward Chapel. Friends may call at Linde Mortuary Sunday from 7 to 9 p.m. and Monday one hour prior to services. Burial will be In Spanish Fork City Cemetery
