I also use genealogybank.com. I guess I did not look deep enough because I did find a Joseph D. Frazier under the Social Security Death Index, but I also found a Joseph David Frazer (same person) under the same index.
I somehow missed the citation notice for him.
I also use Louisiana Cemeteries (http://www.la-cemeteries.com/) to determine locations whenever they may be listed. I find some cemeteries are very useful while others can get hostile (Hope Mausoleum and St. John's Cemetery in Orleans Parish). Fortunately Westlawn is very friendly and helpful. Very nice folks.
I take most of the photos at Westlawn Cemetery over the past few years and very familiar with the cemetery. Using the address you provided I went straight to the grave site. The headstones on either side of the site indicate the correct location, and there was no headstone. The photo I provided is where Joseph Frazer is buried.
When there is no headstone I go to the office and ask them to check their records. I did that in this case. They pulled a card with his burial information and told me that it did not indicate a headstone was there. They indices are by name, not location.
However, since you stated that your husband seems to remember a headstone, I will go back to the cemetery tomorrow and re-verify that I did not miss anything. I will let you know the results.
