HI,You will have to contact the Dept. of Social Services from the link below, It's better to call, I'm not sure what type of information they give now, I was able to receive a relative's patient history years ago.
Have ready, her full name, birth & death dates, her plot number, #456 is what I have on her memorial, they may also ask for other family information, good luck, could you let me know what kind of information is being given out now, I have quite a few people write and ask, would like to pass any new info on to them, Thanks, Karol
