I could go on and on about this, but I think you should consider the plaque very suspect. Obviously this plaque was not a burial that took eight days to complete. There is no stone; this was made on a workbench. The name is spelled incorrectly. The age was 4 hours, not 1 mo or one year.
Use the hospital and the form's information for place and date of both birth and death (the same). As well as being most likely to be correct, that is what someone finding a grave would be searching for. Right?
I do concur with a note about the discrepancy in the biography. Those are really helpful!

Hi,
Just wanted to let you know that I think the plaque is wrong for the Death date of Carl. Both the Oregon Death index and the funeral record I have seen state that his death was the 19 Oct not 13.
You should keep the date of the 13, since that is what it says, but maybe make a note of the above information.
