I've gone and created a handful of duplicates myself, and most involved a deceased wife who took on a husband's last name that I wasn't aware of...it just happens. What's great about Find A Grave is it's not a complicated procedure to fix an error - I ought to know - I've also fudged some edits and links. I now draw out a timeline for most of my memorials, double check any birth certificates, DCs, marriage records, etc., before I commit to it. Even then, I still have a marginal risk.
