On June 20, 1944 Jack was the co-pilot of the North American B-25J Mitchell #43-3961 when it took off in a group of three medium bombers from Pandaveswar, India during an ammunition ferrying flight to Palel, India. After takeoff the crew encountered extreme cloudy conditions and visual contact was lost. It is believed to the plane crashed either due to the weather conditions or mechanical failure. There were no survivors reported. The entire crew was officially declared dead on April 11, 1946.
