LCpl Acosta was killed when a Cobra helicopter gunship and a fixed-wing OV-10 Bronco fired on a platoon of Marines from 3rd Platoon, Delta Company, 1st Battalion,7th Marine Regiment,1st Marine Division.
The pilots were given clearance to fire,when they gave incorrect coordinates for their target (40 NVA in the open) and LCpl Acosta was the only Marine killed by friendly fire. Sixteen Marines were wounded and evacuated to the USS Sancyuary.
