A two-year-old at a nursery school in the UK suffered a cardiac arrest after her neck was wedged between the windows of a playhouse.
Rhiya Malin, 2, from Chigwell, was playing with other children outside the Essex nursery when she rode a scooter into the wooden structure.
An inquest found that nurses were chatting on their mobile phones at the time, and did not notice the accident. A pathologist told the hearing the girl died from a 'reflex cardiac arrest', which can be caused by compression of the neck, and said she could have been saved if help had been given earlier.
