Jack Collis was born in London, Ontario, Canada.

He died while staying in a hotel in Canada; the cause of death is unknown but it's suspected that he died of a pulmonary embolus. Years earlier, he had been ran over by an automobile, which broke both of his legs. This accident could have later caused his death.

Jack had served in the Royal Guards, and even protected the Queen of England on one of her visits to Canada. He later worked as a truckdriver, transporting supplies over frozen lakes in West Canada.

He married Jo-Ann Elizabeth Hayden on September 3, 1966.

He's survived by his daughter D'Anne Elizabeth Collis. He also has a daughter, Marina, of Niagra Falls and a grandson JC. His sisters: Nancy Collis, and Janet Rodrigues live in London, Ontario. Another sister, Marian Bryan, lives in St. Joseph, Missouri. His father, Jack Collis, passed away in 1990. His mother, Florence Collis, passed away in 2003.

He left his family with many warm memories and is greatly missed.
